To access Colette Bessell NDIS Plan Management, all you need to do is ensure you have plan management built into your NDIS plan under the CB Improved Life Choices budget. If you are currently NDIA or Self Managed, you can request at anytime to change your budgets to Plan Managed. You can request this through your NDIS Planner or LAC, your Support Coordinator or you can complete a 'Change of Situation or Details' form and submit to the NDIS yourself. This form can be accessed here or from the forms section of this website.
If you already have Plan Management built into your plan and have a CB Improved Daily Living budget you are able to start receiving Plan Management services from Colette Bessell NDIS Plan Management. Simply complete our referral form with your required details, attach a copy of your NDIS plan and hit send. This information will automatically be sent straight through to our intake team and we will have your plan set up and ready to go the same business day. A welcome pack will be emailed out to you once everything has been set up which contains all the information you need to understand NDIS Plan Management processes and also details to inform your providers where to send their invoices and service agreements. If you are currently Plan Managed by another agency and would like to make the switch to Colette Bessell NDIS Plan Management, simply make the request with your Support Coordinator or you can email your current Plan Manager and request to cease services with them. Typically there will be end of service period outilined in your service agreement so it may take a few days or weeks for your current Plan Manager to end their bookings allowing me to be able to take them over. This varies with each provider. If you do not feel comfortable requesting to cease services with your current provider, please call or email and I can assist you to make the change. A request to change to Colette Bessell NDIS Plan Management form can be accessed here or on the forms section of this website. Once completed and signed I can then make the request on your behalf.
